Bolingbrook Yard - On or about September 14, 2021, the employer failed to ensure an energy control procedure was developed and documented for the control of hazardous energy when employees were engaged in the service and maintenance task of replacing Welsch Ready Mix dump truck air bags, thereby exposing employee to hazards associated with gravitational energy and the unexpected movement of the truck trailer.

An employee sustained a crushed left hand and right index fingertip amputation while replacing a Welsch Ready Mix dump truck air bag.

The employer did not contact OSHA within 24 hours to report the work-related injury that resulted in an amputation.
