29 CFR  1926.304(d):Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe  (a)  New residential construction; Front Side; Lot 19: On or about 9/30/2021, and at times prior, the employer exposed employees  to amputation hazards in that, employees were cutting framing materials with a Makita (Model - 5007F, ser. #245525K) 7 1/4-inch circular saw while the guard was lodged open using a wood shim.  (b)  New residential construction; Front Side; Lot 19: On or about 9/30/2021, and at times prior, the employer exposed employees  to amputation hazards in that, employees were cutting framing materials with a Makita (Model - 5007F, ser. #98303K) 7 1/4-inch circular saw while the guard was lodged open using a wood shim.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  a) New residential construction; Lot 19; Roof:  On or about 9/30/2021, and at times prior, the employer exposed employees to fall hazards, in that a complete fall protection system was not provided to an employee who was conducting framing activities on a 12:12 pitched roof at heights measured at 21 feet 8 inches to the lower level.
