Jobsite:  The employees who were required to use pneumatic nail guns to install roofing shingles, were not equipped with and required to wear eye protection.

Worksite:  The employer did not ensure that each employee who was working from the roof of a commercial building under renovation was protected from fall hazards above (6) feet by the use of guardrails, safety nets, or personal fall arrest systems.
