OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment that were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to being struck by the load. a) On or about September 30, 2021, and October 1, 2021, designated crane operator employees were performing multiple tasks, such as operating crane A, operating Blanker Presses, taking paperwork to other areas of the department, while employees were not removing Crane A control transmitter box from around their body and traveling distances greater than 53'9" with Crane A's control transmitter box in the on position and the suspended load no longer visible to the operator. b) On or about October 1, 2021, designated crane operator employees were not clear of the load before restarting the Crane A to avoid swinging the load as the lift is started.

The employer shall require that the operator avoid carrying loads over people. a) On or about September 30, 2021, the employer did not ensure that designated crane operators did not carry or suspend loads over themselves while cleaning up coil strapping from under the load.
