Employees in this condition were exposed to potential impact injuries caused by falling tools and/or materials.

Employees performing work from the ladder jack scaffolding system in this condition were exposed to falls of 14.6 feet to the ground below.

29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) Jeff Wallick LLC on or about October 04, 2021, at the site location of 7218 Lighthouse Way Perrysburg, OH; The employer failed to ensure employees were trained by a qualified person in the subject matter to recognize and avoid the hazards associated with the ladder jack scaffold use. Unrecognized hazards included but were not limited to; ineffective fall protection use, inadequate supporting structures and overhead protection.
