On or about October 4, 2021, On various locations of the worksite, employees performed steel erection work from aerial lifts without adequate fall protection. A) On the Northside of the job site employees applied steel members from an aerial lift without fall protection. B) On the Northwest side of the job site employees applied steel members from an aerial lift without fall protection.
