29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  The employer is failing to protect employees from electrical shock and electrocution hazards when upgrading lighting fixtures throughout the facility.  This was most recently documented on October 3, 2021 at the jobsite located at 19571 Whitfield Road, Sedalia, Missouri.   An employee working on two 277-volt energized circuits, which had not been rendered inoperative or guarded by insulation or other means, was shocked resulting in fatal injuries.
