29 CFR 1926.451(c)(2)(v):Forklifts were used to support scaffold platforms without ensuring the entire platform was attached to the fork. On or about October 5, 2021, at a commercial construction worksite, framing employees worked approximately ten (10) feet and more from the ground in a scaffold platform attached to a JLG rough terrain fork lift truck that was not secured to prevent the scaffold platform from falling, exposing employees to fall and/or crush by hazards.

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about October 5, 2021, at a commercial construction worksite, the employer did not ensure framing employees' performing framing (fascia) installation work from a scaffold platform utilized fall protection. Employees' were exposed to a fall hazard of approximately ten (10) feet to the ground below.
