29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  Los Amigos Romero y Fuentes LLC worksite located at 1660 Union Park Circle (lot #29), Marion, Ohio 43302:  On or about October 5, 2021, the employer had not implemented an accident prevention program  to identify and mitigate work site hazards which assists employees in maintaining a safe working environment. Employees were exposed to potential fall hazards while performing roofing work and while using a ladder to access the roof of a residential structure.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Los Amigos Romero y Fuentes LLC worksite located at 1660 Union Park Circle (lot #29), Marion, Ohio 43302:   On or about October 5, 2021, the employer did not ensure employees exposed to fall hazards while performing roofing work on a two story residential structure had received training in order to identify and mitigate fall hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Los Amigos Romero y  Fuentes LLC worksite located at 1660 Union Park Circle (lot #29), Marion, Ohio 43302:   On or about October 5, 2021, the employer did not ensure employees were protected from falls while installing ridge vents and shingles  on the second story of a residential structure. The employees were exposed to a fall distances of approximately  10 to 19 feet from the roof's edge.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Los Amigos Romero y  Fuentes LLC worksite located at 1660 Union Park Circle (lot #29), Marion, Ohio 43302:   On or about October 5, 2021, the employer did not ensure the side rails of an extension ladder being used on the front of a residential structure were extended at least three feet above the landing surface. Employees were exposed to fall hazards of approximately 10 feet  while using the ladder to access and egress the roof.
