29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   Worksite: The employee who was required to work with and in close proximity to an approximately 8 foot tall concrete form cage, was not protected from head injuries by the use of protective a helmet.  On or about October 5, 2022, an employee sustained serious injuries after being struck by a fallen concrete form cage.
