29 CFR  1915.114(c): Straps, shackles, and the beam or overhead structure to which a chain fall or pull-lift is secured shall be of adequate strength to support the weight of load plus gear. The upper hook shall be moused or otherwise secured against coming free of its support.  Location: 100 Military Ocean Terminal Street Bayonne, NJ 07002 - Valve Mechanics Shop  a) Employee exposed to crushing hazards while performing inspection and cleaning operations on a 30" butterfly valve utilizing a manual 1 ton Duff Lynx chain hoist with a missing locking latch. The hazard occurred on or about 10/8/21.

Location: 100 Military Ocean Terminal Street Bayonne, NJ 07002 - Warehouse valve shop  a) Employees exposed to crushing hazards while performing valve inspection and cleaning operations on a 30" Butterfly Valve, hoisted on a mobile gantry crane that rolled on an uneven work surface and fell. The incident occurred on or about 10/8/21.
