On or about October 13, 2021, at a work-site located at 2208 Warren Rd., Lakewood, Ohio 44107, the employer did not ensure employees working at the ground were protected from possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   John Cseh was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(c), which was contained in OSHA inspection number 1520430, citation number 1, item number 1, and was affirmed as final order on May 28, 2021, with respect to a workplace located at 1923 W. 54th St., Cleveland, Ohio 44102.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a.) On or about October 13, 2021, employees engaged on roofing activities at 208 Warren Rd., Lakewood, Ohio 44107, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   John Cseh Roofing was previously cited for a violation of this occupational safety and health standards or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection #1520430, citation 1, item number 2, and was affirmed as a final order on May 28, 2021, with respect to a workplace located at 1923 W. 54th St., Cleveland, Ohio 44102.
