29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 14, 2021, employees were exposed to a struck by hazard while performing siding activities on a residential home located at 13237 Ridge, North Royalton, Ohio 44133.  Employees were working on the roof of the home above employees on lower levels with tools and materials that could have fallen on the lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

29 CFR  1926.102(a)(1): 29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  On or about October 14, 2021, employees were exposed to a struck by hazard while performing siding activities on a residential home located at 13237 Ridge, North Royalton, Ohio 44133.  An employee was using a circular saw to cut siding materials and the employer did not ensure that he was wearing protective safety glasses.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:   On or about October 14, 2021, employees were exposed to an electrical shock hazard while performing siding activities on a residential home located at 13237 Ridge, North Royalton, Ohio 44133.  An employee was using a circular saw to cut siding materials, the saw was being powered by a portable extension cord that was plugged into a 110v outlet and the employer did not ensure that the cord was not worn or frayed.

Employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
