Employees were working on the roof of the home above employees on lower levels with tools and materials that could have fallen on the lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

An employee was using a circular saw to cut siding materials, the saw was being powered by a portable extension cord that was plugged into a 110v outlet and the employer did not ensure that the cord was not worn or frayed.
