29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) Timeless Frames, 1 Fisher Circle, Watertown, NY. Mill Area. On or about 10/20/21, an Authorized Employee was performing maintenance activities on a Unimak Gold 95 Molder (100 psi). The employer did not develop, document and utilize energy control procedures for the pneumatic source of energy for machines including, but not limited to the Unimak Gold 95 Molding Machine.   b) Timeless Frames, 1 Fisher Circle, Watertown, NY. Mill Area. On or about 10/20/21, An Authorized Employee  performing maintenance on the Unimak Gold 95 Molder (440 volt) did not utilize the procedures for the control of potentially hazardous energy (electrical).    Abatement certification must be submitted for this item.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.   a)  Timeless Frames, 1 Fisher Circle, Watertown, NY. Mill Area. On or about 10/20/2021, an Authorized Employee was performing maintenance on the Unimak Gold 95 Molder Machine powered by both electrical and pneumatic energy sources.  The pneumatic energy source was not locked or tagged out prior to beginning maintenance activities on the machine. The employee(s) were not trained in the recognition of applicable energy sources (pneumatic), the type and magnitude of the energy available in the workplace and the methods and means necessary for the energy isolated.   Abatement certification must be submitted for this item.

Mill Area. On or about 10/20/21, Prior to providing maintenance on the Unimak Gold 95 Molder Machine, an Authorized Employee did not lock out the energy sources(s) per the Energy Control Procedure.  The employee was not provided with retraining on the use of energy control procedures.  Abatement certification must be submitted for this item.
