29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) Stonebridge Elementary School, 12750 Bedrock Blvd, Spanish Fort, Al.; On or about October 19, 2021: The employer exposed employees to fall hazards of up to 9 feet to the ground below in that the employer failed to ensure employees working from a JLG Lift # MLB9994 had a lanyard attached to their harness and connected to the basket anchor.
