29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye / face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  At building #27, Units 191-198, Josslyn Lane, Jacksonville, FL: On or about 10/20/2021, the employer exposed employees to eye injuries by flying debris, in that two employees using pneumatic nail guns were not wearing safety glasses.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a.  At building #27, Units 191-198, Josslyn Lane, Jacksonville, FL: On or about 10/20/2021, the employer exposed employees to a 10-feet 5-inch fall hazard to the concrete slab below, in that fall protection was not utilized by employees while securing floor joists when walking/standing on the floor joists.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.  At building #27, Units 191-198, Josslyn Lane, Jacksonville, FL: On or about 10/20/2021, the employer exposed employees to a 10 feet fall hazard, in that the Werner Aluminum extension ladder used by employees to access the upper level did not extend at least 3-feet above the landing surface.
