29 CFR  1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about October 20, 2021, at the jobsite, the employer did not ensure employees were in use of eye protection while using pneumatic nail guns and hammers to install roof materials to the roof of a multi-family building.   This exposed the employees to struck-by hazards.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about October 20, 2021, at the jobsite, the employer did not ensure employees were in use of fall protection while installing roofing materials to the roof of a multi-family residential building.  This exposed the employees to a fall hazard greater than six(6) feet to the concrete below.
