29 CFR 1926.1153(h)(1)(i): The employer did not make medical surveillance available at no cost to the employee, and at a reasonable time and place, for each employee who was required under this section to use a respirator for 30 or more days per year: a) Building 200 in rear parking area: The employer did not make medical surveillance for each employee who was required under this section to use a respirator for 30 or more days per year. Employees were required to wear 3M N95 respirators for approximately four months when operating a HILTI cutoff saw with vacuum attachment and chiseling demo hammer with vacuum attachment when repairing building's fa�ade. Employees had an airborne concentration of respirable crystalline silica exposure TWA of 277.5 ug/m3 and 925.0 ug/m3. Condition noted on or about 10/27/2021. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
