At the construction site, the employer did not ensure that employees were wearing hardhats while working below other employees. Employees were exposed to a struck-by hazard from falling objects.

At the construction site, the employer did not ensure that employees using a nail gun for framing activities were provided eye protection.  Employees were exposed to being struck by flying objects.
