29 CFR  1910.502(f)(2)(i): The employer did not provide a respirator to each employee who had exposure to a person with suspected or confirmed COVID-19 in accordance with 29 CFR � 1910.134:  On or about October 27, 2021, and at times prior, the employer did not ensure that employees required to utilize N95 respirators due to positive staff and resident cases of COVID-19 used the respirators in accordance with 1910.134 in that fit testing, medical evaluations, and training on the respiratory protection program had not been conducted. Additionally, employees were provided and required to wear non-NIOSH approved N95 masks.
