29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Worksite located at 1001 N. Burlington Ave., N. Kansas City, Missouri- Employees were working directly beneath overhead hazards that could strike the employees who were not wearing any means of head protection.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:   Worksite located at 1001 N. Burlington Ave., N. Kansas City, Missouri- Employee was exposed to a fall hazard while working out of the scissor lift (mobile scaffold) and was not protected from falling by the use of a guardrail system or personal fall arrest system.
