29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  (a)  On or about October 27, 2021, an employee was conducting carpentry operations from the working level of the scaffold platform that was not fully planked between the front uprights and the guardrail supports. This exposed an employee to a fall hazard of approximately 11 feet.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level:  (a)  On or about October 27, 2021, an employee conducting carpentry operations from a supported scaffold was not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 11 feet.
