29 CFR  1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a)  Jobsite:  On or about October 25, 2021, an employee was working from a ladder jack scaffold and he was not protected to the ground.  The fall hazard to the employee was about 13.5 feet.

29 CFR  1926.503(c):The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  (a) Jobsite:  On or about October 25, 2021, the employer needs to retrain his employees in when and how to use fall protection when working from scaffolding.
