29 CFR  1910.146(f)(7):The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered:  a) Kimberly-Clark paper mill, clarifier area wet well; On or about October 20, 2021 and at times prior thereto the employer exposed employees to confined space hazards in that the employer allowed employees to enter a confined space without first ensuring that the entry permit utilized for the permit required confined space described all hazards such as falls, an unsafe industrial ladder and deep water (30-40ft) present in the permit required confined space.

29 CFR  1910.146(f)(8):The entry permit that documented compliance and authorized entry to a permit space did not identify the measures used to isolate the permit space and to eliminate or control permit space hazards before entry:  a) Kimberly-Clark paper mill, clarifier area wet well; On or about October 20, 2021 and at times prior thereto the employer exposed employees to confined space hazards in that the employer allowed employees to enter a confined space without first ensuring that the entry permit utilized for the permit required confined space described the measures and means used to mitigate hazards in the permit required confined space such as falls, an unsafe industrial ladder and deep water (30-40ft)  present in the permit required confined space.
