29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   Jobsite:  24404 Carter Lane, Foristell, MO 63348.  On or about June 9, 2021:  Employees were engaged in residential water well installation activities located at 24404 Carter Lane, Foristell, MO 63348.  One employee was hospitalized from an injury.   The employer did not report the hospitalization to OSHA within 24 hours.  OSHA was notified on October 15, 2021 and verified by employer on November 3, 2021.
