On or about Thursday, November 4, 2021 at 47-01 Douglaston Parkway, Little Neck, NY, 11362. Employees working on Pump Jack Scaffolds approximately 12 feet above the lower level installing vinyl siding were not protected from falling to the lower level. ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a.) On or about Thursday, November 4, 2021 at 47-01 Douglaston Parkway, Little Neck, NY, 11362. Employees working on Pump Jack Scaffolds on a residential home were not trained on the use of fall protection. The working level was approximately 12 feet from the lower level. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19
