29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On 6:12 pitched roof, located at or near 13006 Palmetto Bluff Drive in Jacksonville, Florida 32257: On or about November 3, 2021, the employer exposed employees to fall hazards ranging from 6-foot to 18-foot 6-inches, in that fall protection systems were not utilized by employees while installing roof deck sheathing.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:   a. At a worksite located at or near 13006 Palmetto Bluff Drive in Jacksonville, Florida 32257: On or about November 2, 2021, the employer exposed employees to a  6-foot fall hazard, in that employees were allowed  to use the top step of a Werner brand,  6-foot A-frame step ladder to access and egress from the roof trusses, while conducting framing operations.
