29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems:   Employees engaged in inventory, stocking, moving boxes activities at the mezzanine located at the overhead storage area in the back of the store over the cooler by the  shipping department were exposed to fall hazard of approximately 9 feet 11 inches from the edge, and from the walking / working surface of the mezzanine to the ground cement below. Employees were exposed in that a fall protection system was not in place.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in meat cutting/grinding/processing activities in the meat department at the Tenderizer machine were exposed to struck-by and amputation hazards in that the safety switch was not functional at the time of the inspection. The safety switch did not shutdown the machine when the guarding/shield was open. Employees need to open the guard to remove the meat that stuck inside the machine; however it was still running and employees were not provided protection from the rotating blades, and nip points.
