29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   (a)  November 1, 2021 and at times prior, workers were exposed to an approximate 25 foot fall from a 3:12 pitched roof while laying water proofing sheets without any means of fall protection.   Note: Abatement certification is required for this item.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a)  November 1, 2021 and at times prior, workers were exposed to an approximate 15 foot fall from the eave of a 3:12 pitched roof while accessing the roof from a ladder that had not been properly extended to allow for ease of access to the roof.
