29 CFR  1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures:  a) On or about November 5, 2021, workers were exposed to electrical hazards while operating a capstan hoist foot pedal while the cord bushing was not properly gripping the outer cover of the power output cord.  b) On or about November 9, 2021, workers were exposed to electrical hazards while operating a capstan hoist motor while the bushing of the cord end fitting was not properly gripping the power supply cord outer cover.
