The employer is failing to protect employees from fall hazards associated with conducting grouting operations on a residential construction jobsite. This violation was most recently documented on or about October 6, 2021 at a jobsite located at 1221 N 170th St., Omaha Nebraska. The employer had employees performing grouting activities on precast concrete members where an unguarded floor hole with a 12' fall to the lower level of the structure was present.
