29 CFR  1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  JOB SITE:  Phillip 66 Service Station and Convenience Store, 301 Pearce Boulevard, Wentzville, MO 63085, on or about 11/8/2021, a worker was observed on a ALTEC boom lift, S/N: 57-108700, mounted on  a Ford F450 Truck, Kansas State License plate # 238 MWB, in an elevated height of approximately 16 feet, 9 inches above ground level.  The worker was not wearing any form of fall protection such as a body belt with lanyard nor a full body harness with lanyard.  The worker was exposed to a fall hazard.
