29 CFR  1926.651(a):Surface encumbrances. All surface encumbrances that were located so as to create a hazard to employees were not removed or supported, as necessary, to safeguard employees.  Job Site: County Road 381, Monroe City, Missouri.  On or about November 8, 2021 the employer failed to support the petroleum pipe line properly to protect the workers who were spray coating the pipeline in order to prevent corrosion.  On November 6 2021, the pipeline that was supported by 22 inch high wood cribbing fell off the support and pinned one of the workers in the excavation.
