29 CFR  1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced;  a) Receiving floor: On or about November 19 2021, and times prior to, the employer exposed employees to falls of 10ft from two (2) 10ft mobile ladder stand platform in that the wheel/casters and rubber pads were damaged which posed a tip-over hazards to employees.

29 CFR  1910.37(b)(5):Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet);  a) Office: On or about November 10 2021, and times prior to, the employer exposed employees to fire hazards, in that a door was not identified and labeled as "Not an Exit" for employees that need to know the egress available in case of an emergency.
