On a 6:12 pitched roof, at a temporary residential construction site located at, or near, 1803 Carter Landing Boulevard, Lot 90, Jacksonville, Florida: On or about November 10, 2021, the employer exposed employees to hazards ranging from 6 to 17-foot, in that fall protection was not utilized by employees while conducting framing operations.

At a temporary residential construction site located at, or near, 1803 Carter Landing Boulevard, Lot 90, Jacksonville, Florida: On or about November 10, 2021, the employer exposed employees to fall hazards ranging from 6 to 10-foot, in that a 28-foot, two-section aluminum extension ladder was separated into two individual sections and each individual section was then used as a single rail ladder to access/egress the roof,  a purpose other than the purpose for which the ladder was designed.

At a temporary residential construction site located at, or near, 1803 Carter Landing Boulevard, Lot 90, Jacksonville, Florida: On or about November 10, 2021, the employer exposed employees to slip, trip and fall hazards, in that the employer allowed employees to use the top step of a 6-foot step ladder to gain access to trusses.  b.  At a temporary residential construction site located at, or near, 1803 Carter Landing Boulevard, Lot 90, Jacksonville, Florida: On or about November 10, 2021, the employer exposed employees to  slip, trip and fall hazards, in that the employer allowed employees to use the top step of a 4-foot step ladder to gain access to trusses.
