29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   a.  Restaurant - On or about November 16, 2021, an ice cream freezer and an extension cord with a microwave and toaster oven plugged into it was plugged into a relocatable power tap (surge protector).   Relocatable power taps are design for low wattage and low amperage appliances.  The employees were exposed to a fire hazard.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  a. Restaurant - On or about November 16, 2021, extension cords were used as permanent wiring. A yellow 3-wire extension cord was plugged into a surge protector.  A microwave and toaster oven was plugger into a white 3-wire extension cord that was plugged into the yellow extension cord.  The employees were exposed to a fire hazard.

29 CFR  1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  a. Corner at Microwave and Toaster Oven - On or about November 16, 2021 a portable fire extinguisher was not mounted. The fire extinguisher was sitting in a corner on the floor.
