29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Worksite, 115-119 Dune Rd. Westhampton Beach, NY- A bicycle bracket for a scaffold was mounted between two interior wall studs and into a horizontal 2"x4". The 2"x4" was not strong enough to hold the weight and the bracket broke from its mount; on or about 11/11/21. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person. a) Worksite, 115-119 Dune Rd., Westhampton Beach, NY- An employee stepped on to an exterior scaffold and the scaffold bracket mount did not have adequate strength to support the scaffold and broke from the wall it was attached to. The scaffold was not erected under the direction of a competent person qualified in scaffold erection; on or about 11/11/21. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
