29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  1087 Center St., Ludlow, MA: On or about November 16, 2021, an employee was observed working on the roof of a residential structure more than 13 feet above the next lower level and was not protected from falling by means of guard rails systems, safety net systems or a personal fall arrest system.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR  1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  1087 Center St., Ludlow, MA: On or about November 16, 2021, an employee was observed using a portable ladder to access and upper surface and the ladder did not extend at least 3 feet past the upper or landing or have grab bars and the risers lashed to prevent displacement.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
