29 CFR 1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a. A Saw Operator was exposed to a noise dose of 123.3%, or an equivalent dBA of 91.5 dBA, which was capable of causing permanent hearing loss. The sampling was performed for 475 minutes during one shift on November 23, 2021. The employer did not have a continuing, effective hearing conservation program.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles may have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 2.44 m (8.0 ft) above the floor may have exposed terminals. a. Building #2: On or about 11/17/2021, the employer exposed employees to contact with energized metal parts, in that a faceplate was missing from an electrical receptacle where employees plugged in extension cords.
