29 CFR  1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On November 17, 2021. Employees were exposed to a fall hazard when working on the 2nd level of a scaffold and using side panels as means of access and egress.

29 CFR  1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:  On or about November 17, 2021 employees were exposed to a fall hazard by working on a scaffold with open ends without a personal fall arrest system or guardrail system.  The Douglas House Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1522625, citation number 1, item number 4  and was affirmed as a final order on 08/31/2021, with respect to a workplace located at 242 Enchanted Way Princeton, TX 75407.

29 CFR  1926.452(c)(2):Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections were not secured.  On or about November, 17 2021 employees were exposed to a fall hazard while working from a scaffold that was not braced on the back side of the scaffold.  The Douglas House Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.452(c)(2), which was contained in OSHA inspection number #1522625 citation number 01005, item number 5 and was affirmed as a final order on 08/31/21, with respect to a workplace located at 242 Enchanted Way Princeton, TX 75217.
