29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) At 1209 East 72nd St., Savannah, Georgia, on or about November 15, 2021, the employer exposed employees to cave-in hazards in that employees were performing work in an excavation, measuring approximately 6 1/2 feet in depth, that was not inspected by a competent person.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) At 1209 East 72nd St., Savannah, Georgia, on or about November 15, 2021, the employer exposed employees to cave-in hazards in that employees, who were replacing a sewer line at a depth of 6 1/2 feet, were not protected in an adequate protection system, such as, but limited to a trench box.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) At 1209 East 72nd St., Savanah, Georgia, on November 15, 2021, the employer exposed employees, who were replacing sewer lines in the excavation, to struck-by hazards in that the spoil pile was not placed at a distance of greater than 2 feet from the edge of the excavation.
