29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye and face protection when exposed to eye and face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Indoor Tennis Courts:  Employer did not provide and ensure the use of eye and face protection to provide protection against splash hazards from hazardous cleaning products such as chlorine bleach-based cleaners, Chlorox Tilex Mildew Remover, and RMR-86 Mold Stain & Mildew Stain Remover which also contained sodium hydroxide while employees clean the ceiling.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  Establishment:  Employer did not develop and implement a Hazard Communication Program for employees who were exposed to hazardous materials such as chlorine bleach-based cleaners, Chlorox Tilex Mildew Remover, and RMR-86 Mold Stain & Mildew Stain Remover which also contained sodium hydroxide while cleaning the ceiling and walls of the indoor tennis courts that included the following elements:  a) Employee training in the hazards of the material they handle and measures that employees can take to protect themselves from these hazards.  b) Labelling. c) Safety Data Sheets. d) Non-routine tasks. e) Outside contractors. f) List of hazardous materials on site.
