29 CFR  1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a-8 Brookside Ave, Suffern NY 10901: An employee engaged in roofing activities using a pneumatic nail gun was exposed to eye los/eye injury due to struck by hazards as eye protection was not utilized. Condition observed on or about 11/22/2021

29 CFR  1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:   a-8 Brookside Ave, Suffern NY 10901:  Employees engaged in roofing activities on a 6 on 12 pitched roof were exposed to fall hazards of approximately 10 & 18ft t to the asphalt below in which a means of fall protection had not been utilized. Condition observed on or about 11/22/2021
