29 CFR  1910.132(d)(1)(i):Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment.  a. On or before November 22, 2021, the employer did not select, and require employees to use, Personal Protective Equipment that would protect employees from the hazards identified in the hazard assessment. An employee was stirring the mold when a molten metal and slag eruption occurred and the employee was not wearing a neck cape, aluminized spats, and specialized thermal gloves, thereby resulting in the hospitalization of the employees due to 3rd degree burns.  b. On or before November 22, 2021, the employer did not select, and require employees to correctly use, Personal Protective Equipment that would protect employees from the hazards identified in the hazard assessment. Two employees were standing in the area identified as the Red Zone, with the employees' face shields in the raised position, thereby exposing the employees to struck-by hazards from sparks and molten metal.
