29 CFR  1910.266(h)(1)(vi): Each danger tree was not felled, removed or avoided:   a) On or about 7/20/2021 at a logging site in the woods located near 8177-8337 Nettle Hill Road Sherman, New York; an employee was felling trees without clearing or avoiding danger trees in the work area. Trees were being felled within two tree lengths of danger trees, including trees being felled which were  coming into contact with  standing danger trees.      ABATEMENT DOCUMENTATION  REQUIRED

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye;  a) On or about 07/20/2021, where an employee hospitalized due to injuries from a falling tree limb. The employer did not notify OSHA of the in-patient hospitalization. OSHA became aware of this incident and subsequent hospitalization on 11/18/2021.  ABATEMENT CERTIFICATION REQUIRED
