On or about November 22, 2021, at the site location of Oak Hill Apartments Maumee, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as but not limited to falls during job site operations.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Professional Construction & Remodeling LLC. On or about November 22, 2021, at the site location of Oak Hill Apartments Maumee, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing activities. Employees in this condition were exposed to falls over 21 feet.

In this condition employees were o exposed to falls over 21 feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Professional Construction & Remodeling LLC. On or about November 22, 2021, at the site location of Oak Hill Apartments Maumee, OH: The employer did not ensure the side rails of the extension ladder used on site to access the roof were extended at least three feet above the landing surface. Six employees working from atop a residential structure roof were exposed to falls from a height over 21 feet while accessing work area from ladder.
