On or about November 22, 2021, at the site location of Oak Hill Apartments Maumee, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing activities. Employees in this condition were exposed to falls over 21 feet.

On or about November 22, 2021, at the site location of Oak Hill Apartments Maumee, OH: The employer did not ensure the side rails of the extension ladder used on site to access the roof were extended at least three feet above the landing surface. Six employees working from atop a residential structure roof were exposed to falls from a height over 21 feet while accessing work area from ladder.
