(Construction reference 29 CFR 1926.602(d)):  a) Exterior of Building - Front - 3rd Floor Balcony: Employees were exposed to fall and struck by hazards as the All Terrain Forklift was operated by an individual that had not been trained and evaluated in the safe operation of the equipment, on or about 10/27/21.

29 CFR  1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.   a) Interior of Building - Front - 3rd Floor: Employee engaged in window installation was exposed to an approximately 30 feet fall hazard as he connected his lanyard around a ceiling truss and into it self, on or about 10/27/21.
