29 CFR  1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  (a) On or about November 30, 2021, at the construction site located at 10448 N. Port Washington Rd. Mequon, WI 53092, an employee was performing framing activities on a commercial building while in an aerial lift with fall protection system in place but not connected to the boom or basket, and was not protected from falling to the lower level. This exposed an employee to a fall hazard of approximately 8 feet.
