29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about December 1, 2021, employee(s) conducting roofing work on a 4:12 pitch roof on the west side of the detached garage structure, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eight (8) feet four (4) inches.  (b) On or about December 1, 2021, employees conducting roofing  repair work on a 10:12 pitch roof on the west side  of the  main house structure, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9)  feet four (4) inches at the eave.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) West side of detached garage: On or about December 1, 2021, an employee  engaged in roofing work on the West side of detached garage, was using a Werner eight (8) foot step ladder designed to be used in the open, and braced/ locked  position, while the ladder was left in the closed position and leaned against the structure. The employee used the first two steps of the ladder for elevation to perform work.
