29 CFR  1928.57(d)(1)(v): Chains and sprockets shall be completely enclosed, except that they may be guarded by location if the bearings are packed or if accessible extension lubrication fittings are used.  On or about November 26, 2021, employees were exposed to caught in hazards from using a Consolidated 198 Gin stand with an unguarded chain.
